Association of kidney biopsy findings with short- and medium-term outcomes in children with moderate-to-severe IgA vasculitis nephritis

Assessing the initial severity of immunoglobulin A vasculitis nephritis (IgAV-N) is important due to its determining effect on kidney management and outcomes. This paper describes a multicentre paediatric cohort IgAV-N patients discusses relationships among clinical presentation, histological features, outcome. We retrospectively studied 170 children with biopsy-proven IgAV-N, diagnosed between 2007 2017. One-quarter (27%) presented nephrotic syndrome (NS). Kidney biopsy revealed International Study Disease (ISKDC) grade II or III in 83% cases. Endocapillary proliferation was observed 73% patients, chronic lesions were 25%. Data analysis showed significant association NS at onset endocapillary cellular crescents. After median follow-up 21 months (IQR 12–39), 30% had persistent proteinuria decreased eGFR. At end follow-up, impairment more often those crescents biopsy.
